CS Student Forms

1. All links below are downloadable pdf forms and are forms designed with specific fillable pdf text fields.
2. Please download the applicable form for your relevant purpose by clicking on the (link) or form required.
   1. Once, after clicking on the link, the pdf file opens in a new window;
      1. In order to use the pdf document as a fillable pdf form, click on “File”, then
      2. Click on “Save Page As…”, and then
      3. Save the pdf document to a location on your computer with the specific name and your T-number.
         1. For example, DropTNumber or DropT00056789 or WithdrawalT00056789.
   2. Once saved to your computer, open the downloaded pdf document and the pdf document when opened, it becomes a “fillable pdf text form”.
3. Fill out the necessary information requested of you, make sure the information is correct, sign and email the form to Dr. Li, Professor of Computer Science at Liww@tsu.edu
